I consent to participate in the Fountain Teens Summer Camp from 7th August to 19th August 2023, starting at 9 am daily. I commit to attending the camp daily, following all rules, and taking full responsibility for my behavior. In case of emergencies, my emergency contact information is provided. As the parent/legal guardian, I grant permission for my child to attend the camp.
